Official Nomination Form
This form is for nominating business leaders, entrepreneurs, or companies for the 2026 ICOC Minnesota Business Excellence Gala. Nominations can be submitted by anyone, and a separate form is needed for each nominee and category, with nominee consent required. All information will be kept confidential for judging purposes.
