This nomination form is intended for individuals or organizations seeking to nominate women leaders for recognition at the 2026 ICOC Minnesota Women in Leadership Summit. Nominations may be submitted across three categories — Community Impact, Cultural Excellence, and Business Leadership — and may come from family members, friends, colleagues, employees, clients, or community partners. A separate form must be completed for each nomination and award category, and nominee consent is required prior to submission. All information provided will be treated as confidential and used solely for judging and award administration purposes.

All disputes arising under this nomination process will be governed by the laws of the State of Minnesota and subject to the jurisdiction of Hennepin County Courts.
